On the last episode of Jewel in the Palace, Jang Geum successfully returned to the palace with the help of the Queen and was given the freedom doing his profession as a physician. And when she and her family were walking along the beach, they found a pregnant woman who is about to deliver the twin.

Jang Geum found out that there was no option availalble to help the woman but surgery. She asked her husband for the permission to do it and he agreed. After preparing the apparatus needed for the surgery, Jang Geum immediately started the surgery and was successful on it, saving the life of the woman and her twin.

And according to the history, Jang Geum was the first to perfrom surgery.
